These terms cover the Canyon Foot & Ankle minimally invasive foot care page, the request form on it, and the text messages that may follow. By using this page you agree to them.

Information on this page

Everything on this page is general information about foot and ankle care. It is not medical advice and it is not a diagnosis. Reading it, watching the videos, or sending a request does not make you a patient of Canyon Foot & Ankle. Only an exam can tell whether a treatment is right for you, and individual results vary.

Requesting an appointment

Sending the form asks our team to contact you. It does not book an appointment by itself, and it does not guarantee a particular appointment time, doctor, or treatment. Our team will call you to talk through your request and set up a visit.

Please keep medical details out of the form. We will ask what we need to know when we call.

Text messages

Canyon Foot & Ankle Specialists, PC sends text messages only to people who agree to them on the form. The form has two optional boxes, and both start unchecked:

• About your request. If you check the first box, we may text you about your appointment request, including callbacks, scheduling, and appointment updates.
• Foot and ankle education. If you check the second box, we may also text you educational information about foot and ankle conditions and treatment options.

Agreeing to texts is not a condition of receiving care. Message frequency varies. Message and data rates may apply.

Reply STOP to any message to stop receiving texts. We will send one message confirming you have been unsubscribed, and then no more. Reply HELP for help, or call us at (208) 733-0436.

Wireless carriers are not responsible for delayed or undelivered messages. We do not share your phone number or your text consent with third parties for their own marketing. Our Privacy Policy explains how we handle your information.

Email

If you send a request, we may email you about it and send follow-up information about foot and ankle care. Every email has a link to unsubscribe.

Other websites

This page may link to other websites, such as video hosts. We are not responsible for their content or their privacy practices.

Changes

If we change these terms, we will update the date at the top of this page.
