Privacy Policy
This policy covers the Canyon Foot & Ankle minimally invasive foot care page and the request form on it. It explains what we collect, what we do with it, and the choices you have.
What we collect
When you send a request, we collect what you type into the form: your name, mobile phone number, email address, the main concern you choose from the list, whether you plan to use insurance, and the name of your plan if you give it. We also record the date and time of your request and which page you sent it from.
Like most websites, this page records basic technical information such as your IP address, browser, and device type, and it may use cookies and similar tools from advertising and analytics providers (for example Meta and Google) to measure whether our ads are working. These tools do not receive the contents of your request form.
How we use it
- To call, text, or email you about your request and to set up your appointment or consultation.
- To send appointment reminders and follow-up about your care at Canyon Foot & Ankle.
- To keep a record of your request in our practice systems.
- To understand how people find and use this page, so we can improve it.
We do not sell your information. We do not share it with anyone outside the practice except the service providers who run our forms, messaging, and website for us, and they may only use it to do that work.
Text messages
The form has two optional text message boxes, and both start unchecked. If you check the first, Canyon Foot & Ankle may text you at the number you provide about your appointment request, including callbacks, scheduling, and appointment updates. If you check the second, we may also text you educational information about foot and ankle conditions and treatment options. Message frequency varies. Message and data rates may apply. Reply STOP at any time to stop receiving texts, or HELP for help. Agreeing to texts is not a condition of receiving care. We do not share your phone number or your text consent with third parties for their own marketing. See our Terms of Use for the full text message terms.
Your health information
Please keep medical details out of the form and out of text messages. We will ask what we need to know when we call you. Once you become a patient, your health information is protected under the Health Insurance Portability and Accountability Act (HIPAA) and our Notice of Privacy Practices, which you receive at your first visit. Web forms and text messages are not a secure way to send health information.
Your choices
- You can ask us to stop contacting you at any time by replying STOP to a text, by calling us, or by telling anyone on our team.
- You can ask what information we hold about your request and ask us to correct or delete it.
- You can control advertising cookies through your browser settings and through the ad settings offered by Meta and Google.
Children
This page is meant for adults. If you are requesting care for a child, a parent or guardian should send the request.
Changes
If we change this policy, we will update the date at the top of this page.
